Social Media Intake Form

Heya,

As promised, I created and have sent out a few intake forms for my clients, as requested. If you need a social media intake form, use the chat box on the bottom right side of this page. This form only takes a few minutes to complete. It collects your basic contact information as well as generalized questions pertaining to your social media presence. Since this is just an intake form, you will not be asked to provide a payment method at any point in this form provided. As for purchasing a quoted project, Rakelle Ann will formulate an accurate quote as soon as possible.

This intake form is for your social media account. I, Rakelle Ann, am a social media influencer, who specializes in content creation; however, I do not sell direct products. I am a digital content creator. Think of me, as your supportive assistant for your platform. I require all my clients to fill out a Non-Disclosure Agreement (NDA) & proper signatures from third party corporations, vendors or affiliates. Since you were invited to fill out this form, you can message me directly if you have any additional questions. 

Again, this is an intake form, you will receive a quote for services rendered and products when applicable.
